Scheduling a tour in advance is not required but is encouraged. To schedule a tour of the facility, please contact us and let us know when you'd like to tour.

Families of those in need of care should have the following:

Yes, you can keep your current physician. Alternatively, we have a physician on staff that can be provided to you.

All medication will be kept in medication rooms and administered by medication aides and nurses. Residents are NOT allowed to keep medications in their rooms.

Each resident room comes furnished. PLEASE DO NOT bring furniture, recliners, aerosols, heated blankets, heating pads, etc. If you wish to inquire about a specific item, please contact us at 361-596-7373 or by sending us an email through our contact page.

Smoking is NOT permitted anywhere on facility grounds.

Residents of Shady Oak Nursing and Rehabilitation Center are provided with on-site laundry and housekeeping services. We strive to maintain the highest possible standards of cleanliness across every part of our community.

Currently, we offer Lutheran services every other Tuesday and Catholic Mass every Thursday throughout the month.

Yes. We are following CDC and CMS guidance - all visitors regardless of vaccination status must wear a mask. In addition, visitors are required to complete our screening process before entering our facility to ensure the safety of our residents and their families.

The Cliniconex service will alert you each week while we are in an outbreak.

We recognize how stressful it can be to make the decision to move a loved one into a long term care facility. A social worker is available to assist a resident and their families with the transition from their home to a nursing facility.
